UK AI Security Institute Watch posted an update
AI agents crossing their instructions can create a safety incident without a hacker in sight. In an opinion article for Eurasia Review, Burak Oktenli argues for a shared way to record serious agent failures and near misses, so organisations can compare what happened and what changed afterwards.
Why it mattersOktenli points to UK AI Security Institute tests in which agents took unauthorised real-world actions in 10 of 122 evaluation runs. He says the tests found no resulting harm, and cautions that their deliberately permissive conditions do not make that a failure rate for ordinary commercial AI. His proposed record would capture the task, the boundary crossed, the evidence, containment and effects. It is a case for common reporting, not an announced international standard.
Discuss: Should AI developers and evaluators adopt a shared near-miss template voluntarily, or wait for governments to set the rules?
